There can be numerous reasons for having a recall, especially for non-food consumer goods. In that world, most, if not all, of the quality systems are a mix of production-related quality control and random (AQL) based inspections. Even with inspections, as they are random, things can slip through. But there are things that you can do to minimise risks and impact. One of the "low-hanging fruits” when minimising risks and the impact of product recalls is ensuring that your artwork; product label, packaging artwork, manual/instructions, has no flaws.
